Expression of Interest: Please register your Expression of Interest to participate as a Young Leader volunteer peer mentor in our Holiday Program held at Southern Support School, Howrah. A YLOT representative will be in touch with you to discuss.

Transport Permission

Depending on numbers and Young Leader participation, Young Leaders may have opportunities to join the Community Access group during the Holiday Program. Details about the activity and excursion will be provided should your Young Leader participate.

I understand that my child will:
• Be driven to and from program activities and excursions by a representative of Young Leaders of Tasmania (YLOT).
• Travel in a YLOT corporate vehicle or approved staff vehicle.
• Be transported by a driver who:
o Holds a current valid driver’s licence
o Has a registered and roadworthy vehicle
o Provides appropriate seating with seatbelts/restraints
o Holds current Registration for Working with Vulnerable People

Supervision
I understand that my child will:
• Be supervised at all times during program activities and excursions by YLOT staff.
• Be expected to follow all safety instructions and behaviour expectations.

I understand and give permission for transport when required

Media Consent
